Waiver and Release of Liability

I, the undersigned, agree to the following terms and conditions regarding the grooming services provided by Fluff and Puff:

1. Health and Vaccinations: I confirm that my pet is fit, healthy, and fully vaccinated. I will provide proof of vaccinations upon request.

2. Disclosure of Medical Conditions: I acknowledge that any mishap caused by my non-disclosure of my pet's medical condition or behaviour is my responsibility. If my pet's health causes concern, I authorize Fluff and Puff to obtain veterinary treatment at my expense.

3. Stress and Medical Issues: I understand that grooming can be stressful for some pets and in very rare occasions can cause an unforeseen medical issue. In such cases, Fluff and Puff will attempt to contact me; however, if I am unavailable, I authorize Fluff and Puff to seek veterinary care at my expense.

4. Behaviour and Aggression: If my pet bites or attempts to bite any person or pet, a muzzle may be used, or at the discretion of the groomer, treatment may stop. I will pay for all work done up to that point and will cover the full cost of all damages and expenses caused by my pet.

5. Collection of Pet: If I fail to collect my pet at the agreed time, I will pay all extra costs due.

6. Groomer's Liability: I agree that the groomer will not be held liable or responsible for irritation, abrasion, patchiness, or hair loss due to any pre-existing skin condition or as a result of the grooming process, including dematting, thinning, stripping, or shaving.

7. Matting: I understand that if my pet’s coat is matted, it may be necessary to shave the coat down to prevent pain or injury. I acknowledge that grooming a matted pet may cause nicks, cuts, or skin irritation, and that Fluff and Puff is not responsible for these occurrences.

8. Photographs: I authorize Fluff and Puff to take photographs of my pet for use in promotional materials, social media, and on the company website.

9. Cancellation and No-Show Policy: I understand that I must provide at least 24 hours’ notice if I need to cancel or reschedule an appointment. If I cancel with less than 24 hours’ notice or miss my appointment, I agree to pay a fee equal to the full grooming cost at my next visit.

By signing below, I acknowledge that I have read, understood, and agree to the terms and conditions outlined in this waiver. I agree that Fluff and Puff, its owners, employees, and agents are only liable for damages caused by their negligence during the grooming process and are not responsible for any issues arising from pre-existing conditions.

Consent for Data Collection and Use

By providing your personal information, pet's details, and veterinarian's contact information, you consent to Fluff and Puff Dog Grooming collecting, storing, and using this data. This information is essential for us to contact you regarding our grooming services and, if necessary, to reach out to your veterinarian in case of an emergency involving your pet. We are committed to protecting your privacy and will use your information solely in accordance with our privacy policy. We adhere to European laws and the General Data Protection Regulation (GDPR), and your consent is required by law for us to process this information.
